2) Form fields must be filled out ONLINE for legibility. Please do not print it and fill in by hand.
3) We recommend selecting the checkboxes to highlight fields, including required fields.
4) Once registration form is filled out completely:
- PRINT form (print button is located at the bottom of the form)
- Your signature is required twice on Page 2
- Return the signed form & appropriate payment
